System AVP of Care Coordination / Case Management

We’re partnering with a mission-driven health system to identify a System AVP of Care Coordination. This is a high-impact leadership role overseeing case management, social work, transitional care, and discharge planning across multiple hospitals.

Open to permanent or interim (6+ months).

Key Responsibilities

• Lead UR, discharge planning, and care coordination
• Oversee ~120+ FTEs across multiple sites
• Strengthen leadership, accountability, and team performance
• Improve post-acute strategy and transitional care programs
• Support key initiatives including ECM and hospice expansion

What We’re Looking For

• Strong background in case management, UR, and discharge planning
• Experience leading through turnaround and team stabilization
• Hands-on, visible, collaborative leadership style
• Strategic thinker who can partner at the executive level

Requirements

• RN + active California license
• BSN + Master’s (MSN, MBA, or related)
• Magnet environment experience preferred
