Health and Wellness Nurse (LPN or RN)

Our client, a Senior Independent and Assisted Living Community, is looking to hire a full-time Licensed Practical Nurse OR Registered Nurse.  This can be a day shift (8am-4:30pm) or an evening shift (2pm-10pm)!

Duties & Responsibilities include:

• Assist the Resident Care Director with prescreen/admission, assessments, and service plan coordination.
• Audit SAMM for compliance.
• Assist in monitoring of medication administration.
• Supervise and oversee Memory Care and Resident Care Assistants.
• Report illnesses and incidents to appropriate next of kin.
• Assist with pharmacy deliveries.
• Adhere to company policies, procedures and standards of conduct.
• Occasional On Call required.

Requirements include:

• Current RN or LPN license in state of Massachusetts.
• Experience in Assisted Living preferred.
• Ability to lead, direct and supervise others.
• Previous experience working with individuals with dementia including handling complex behavioral issues and educating staff and families on dementia related topics.
• Certification in Dementia Care preferred. Excellent ability to communicate and follow up with residents and family members.
• Strong problem solving and critical thinking skills.
• Proficient MS Office computer skills.
• Must be able to move at least 50lbs without assistance; bend, reach and work in small areas; push and pull equipment; be able stand for long periods of time.
