Our client, an adult day health center, is looking to hire a full-time Activity & Outreach Coordinator. The person in this role will be responsible for overseeing the therapeutic activities program, leading outreach & referral efforts, and participating in company-wide strategic initiatives.
This is a Monday-Friday onsite position split between Acton and Arlington, MA.

Key Responsibilities:
- Develop, plan, and implement the activity and recreation programs to meet the goals and needs of individual participants, and the group needs of all participants.
- Complete all regulatory documentation, such as quarterly activities notes, and care plans, on all participants, in a timely manner.
- Work in tandem with programmatic staff to develop, write and disseminate the monthly activities calendars.
- Contribute to agency-wide projects to expand services and community partnerships; assists the Chief Strategy Director in strategic planning initiatives.
- Work collaboratively with the Director of Philanthropic Investment in seeking out community partnerships and collaborations that maximize resources for the activities programming, including through free and reduced costs services, donations, or scholarships.
- Coordinate and lead special community programs.
- Responsible for daily data entry in electronic records, ensuring documentation is accurate and complete.
- Responsible for receiving and distributing incoming referrals for the agency, coordinating with program service coordinators to ensure referrals are followed up on.
- Responsible for outreach and collaboration with client referral sources to ensure lead generation.
Qualifications & Skills:
- Bachelor’s degree preferred, may be waived in lieu of experience.
- Minimum 2 years’ experience working with the aging population.
- Therapeutic recreation specialist or activity professional certification required.
- Computer proficiency required.
- Ability to work collaboratively as a member of multidisciplinary and cross-functional teams.
- Ability to function as an effective change agent.
